The Texas Allergy, Asthma and Immunology Society (TAAIS) Memorial Foundation is a 501c3 non-profit organization under the IRS code.
The Texas Allergy, Asthma and Immunology Society (TAAIS) Memorial Foundation is pleased to announce the 2022 TAAIS Memorial Foundation Asthma Scholarship Award.
This $3,000 one-time award will be granted to up to five qualified and deserving high school seniors in Texas who suffer from Asthma.
The award will be payable directly to the college or university of choice in the student’s name.

APPLICATION INSTRUCTIONS
Before you begin applying, please read the application requirements and instructions:
Qualified applicants will be seniors attending high school in Texas for the 2021-2022 academic year that suffer from asthma and plan to attend college after high school.
You will need to submit in the form below to be considered:
- Completed Application
- One typed Essay (uploaded as PDF)
In 650 words or less (one typed page maximum), please describe the impact that asthma has had on your life, including its severity. This should include how your asthma has affected your life and how you have dealt with your asthma in school and in other aspects of your life. You will upload the file as a pdf. - Counselor Recommendation Letter (uploaded as PDF)
- Teacher or Community Adult Recommendation Letter (uploaded as PDF)
- Current High School Resume (uploaded as PDF)
Resume should outline your extracurricular activities in school and outside of school, community service activities, leadership positions, work experience, and honors or awards. Please indicate your grade level for each activity. - Physician Report (uploaded as PDF)
